Patient Advocate Geriatric Care Management

As a parent or loved ages, the issues of care giving can become very challenging, involving many complex questions and difficult decisions. Many family members are part of the “sandwich generation” caregivers to both their children and parents.  Often family's are unprepared, overwhelmed and harbor feelings of guilt about not being able to do it all.  These feelings are not unusual, it’s an emotional time for family members, with much to learn and important difficult decisions to be made.

HCC’s compassionate geriatric care managers have the solutions you need.  We provide the knowledge necessary to make informed decisions, allowing you to make the right choices for your loved one. We act as the liaison between the older adult, family members, healthcare providers and the community.

 

Services include:

Geriatric assessment to determine care and needs, then develop and implement the care plan.

Ongoing care management, reassessing the plan as abilities and needs change

Obtain and oversee home health agencies, social and rehabilitation services.

Communicate and coordinate with other professionals, service providers and family members; provide trusted referrals within the community

Collect all medical records, coordinate medical appointments, confer with and facilitate communication among all the physicians, caregivers, home health agencies and hospitals, nursing homes, assisted living and /or rehabilitation facility. 

Research and assist in transitions from home to assisted living, senior communities and skilled nursing facilities, where we will continue to monitor and oversee care.

When hospitalized our certified patient advocates guide the patient through the healthcare system, ensuring the diagnosis, prognosis and treatment options are clearly understood.

Make sure all the caregivers are on the same page, treating the whole person, not just the doctor’s specialty.

Research illness and disease, ensuring you get the education needed so true “informed consent” can be given.


Evaluate discharge plan and refer to appropriate rehabilitation facility. Oversee care plan at rehabilitation facility

Evaluate home and arrange for home care, nursing, therapist, aides, equipment, then manage and oversee the care.

Long Term Care Assistance

 

  • Arranging and monitoring home health services for a loved one, whether you live nearby or across the country.
  • Visiting any facility treating a family member and giving you peace of mind that appropriate care is being given.  You may live far away, but we'll visit the facility for you. 

  • Provide "feet on the ground" for long distance and sandwich generation caregivers, by managing your loved one's healthcare at home or in a senior residence.
  • Clarifying and coordinating treatment options with doctors and caregivers.  We’ll make sure you understand the medical and financial issues.
  • Reviewing any bills doctors, nurses or other providers send to ensure all insurance benefits are being paid at the proper rate.
  • Working with you and caregivers to focus on rehabilitation and follow up care.
  • Working with you or a loved one from the beginning of any facility stay to setting up new continuing care arrangements.
