How it Works
A professional geriatric patient advocate conducts an initial assessment with the senior and his/her family. Care is taken to determine the wishes and needs of the client and family. The same advocate will continue to monitor monthly, coordinating information and facilitating communication between all parties, including all healthcare providers.
Who is it For?
The ideal candidates are those who are not compromised in their daily activities or functioning, but who may be starting to experience health concerns, as well as those whose families live long distance or are in the "sandwich generation" and need the peace of mind that comes with knowing a professional is monitoring their loved ones' care. For residence of Assisted Living or Skilled Nursing Facilities, our professional geriatric care managers provide an additional layer of supervision and coordination of care, especially when outside health-care providers are involved.
The Co-ordicare Program includes:
Assessment and planning
Monthly visits
Review of medication and update medical history
Referrals to trusted local resources and services
Screen and monitor in home health agencies
Updates to family members
